Help people living with cancer navigate every step of their pathway — from diagnosis, through treatment and into person‑stratified follow‑up — by joining Walsall Healthcare NHS Trust as a Cancer Care Navigator based at Manor Hospital, Walsall (WS2 9PS). You will work closely with Clinical Nurse Specialists, MDTs and cancer trackers to coordinate care, deliver Holistic Needs Assessments and telephone care‑planning clinics, support remote monitoring and PSFU pathways, manage the Somerset Cancer Registry and ensure investigations and clinical alerts are actioned to keep patients safe and prevent loss to follow‑up. Daily tasks include telephone triage, providing level 1 psychological support within agreed protocols, accurate electronic record‑keeping, liaising with internal teams and external partners, advocating for patients and carers, and contributing to education and health‑and‑wellbeing programmes.
