National Clinical Lead for Primary and Community Care; 0.4 wte / 4 sessions The purpose of the role is to provide clinical leadership for the redesign of components of the primary care system to support whole system improvement in the quality, safety, improvement and performance of local primary healthcare services, integrated with secondary care, and with services from local government and the third and independent sectors. This work will reflect the Primary Care Model for Wales and the work of the Strategic Programme for Primary Care. Cluster Lead Advisor to the Strategic Programme for Primary Care (Clinical or non-clinical; current Cluster Lead in Wales); 0.2 wte / 2 sessions The SPPC recognises and values Cluster leadership and is committed to the further development and delivery of the Primary Care Model for Wales building on this local leadership and expertise. In collaboration with Assistant Medical Directors and Heads of Primary Care, the post holder will: Provide professional direction in facilitating the rebalancing of resources from Secondary Care to Primary Care. Provide a dedicated national primary care clinical leadership resource to consider national care pathways, enhanced services and enable management of place based care closer to home and support implementation of the Primary Care Model for Wales. Lead packages of work on behalf of the Strategic Programme for Primary Care. Provide professional advice and challenge to the Strategic Programme for Primary Care and Assistant Medical Directors and Heads of Primary Care Peer Groups. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. The Strategic Programme for Primary Care, part of the NHS Wales Executive, hosted by Public Health Wales, is an All-Wales Health Board-led programme that brings together and is developing all previous primary care strategies and reviews at an accelerated pace and scale, whilst addressing emerging priorities highlighted within A Healthier Wales . Based in the NHS Executive, the purpose of the post is to provide clinical leadership for the redesign of components of the primary care system to support whole system improvement in the quality, safety, improvement and performance of local primary healthcare services, integrated with secondary care, and with services from local government and the third and independent sectors.
