To act as an autonomous nurse taking on going responsibility for the care coordination of appropriate patients.2. Be responsible for the assessment, planning and implementation and evaluation of nursing care to individuals, groups of patients and their families on the district nursing caseload encompassing a rehabilitative and self-care approach delegating activities as appropriate.3. To act as the practice link with GPs-in and Out of Hours and other general practice staff.5. Demonstrate, teach and supervise the nursing care that can be safely delegated to the patient/carer giving support as necessary.7. You will have extensive clinical experience and experience of case management, preferably within the community setting and be able to demonstrate recent and ongoing clinical professional development relevant to this post.
