This role is perfectly suited to someone with experience of working with carers of people experiencing difficulties with their mental health. Based in the Early Intervention in Psychosis Team/Community Mental Health Team you will work closely with other members of the Multi-Disciplinary Team, in leading the assessment and provision of support to carers, family members and significant others to enable their mental health and resilience to be optimised. We are seeking a dynamic, innovative, enthusiastic individual who is committed to working with carers within the community setting. As a Carer Support Worker you will identify carers coming into the service and will ensure they have been offered a Carers Assessment as per Section 75 delegated responsibility from the Local Authority. You will lead on the completion and inputting of carers assessments, following the process through to payment where identified and keeping a track of those requiring annual reassessments. An integral part of your role will be to ensure that the role of unpaid carers is recognised, supported and valued, ensuring that carers are well informed, supported with their individual health and well-being needs and involved in decision making about care in relation to their caring role. You will provide carers with information and advice about services both within and external to the organisation, that contributes to their caring role and increases their knowledge regarding care and treatment processes. You will also signpost and facilitate carers access to a range of support services.
