The Bordon and Petersfield Community Mental Health Team operates from Petersfield hospital where this is our main site. The CMHT provides services to patients in the Bordon and Havant area as well . The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues. We support our staff in developing their clinical skills in attending post graduate training such as physical health and nurse prescribing. Due to the rural nature a car driver is essential with access to own car. We would consider the base being Havant with he expectation that you would travel to Petersfield and Bordon To provide recovery based community mental health services within a multidisciplinary team to adult individuals with severe and enduring mental health needs referred to the Community Mental Health Team. This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings.
