The focus of the role will be holding a complex caseload and delivering clinical Occupational Therapy to service users in Home Treatment Team and to liaise with Teams in Urgent Care. To work closely with multi professional colleagues particularly nursing colleagues to develop and embed the integrated clinical model; in particular to enhance opportunities for the delivery of occupational therapy with a focus on achieving to support transitional care, recovery and effective discharge planning and improving physical health. Essential criteria include a keen interest, clinical experience, leadership, advanced communications skills and a flexible approach is prerequisite. You will need to have a vision to implement service improvement initiatives, to drive the services forward as part of the Home Treatment Team. You will possess the ability to work autonomously, multitask and be self-organised, promoting further development and influence high quality clinical practice. You will be responsible for supporting the delivery of high standards of safe patient care in line with National guidance. You will monitor and implement local management and treatment protocols and pathways. In return we will be committed to supporting your professional training needs and personal development plan. The successful candidate will receive a good opportunity to develop a range of skills within a varied portfolio, with the support of an existing network of supportive and experienced Lead Occupational Therapist, matrons Ward Team managers. The successful candidate is required to work full time-30 hrs and will be included in the Trust on call cover, training will be provided. The post covers services that are spread across the city.
