The service will be developing into the Safe at Home team, which will aim to prevent hospital admission by managing acutely deteriorating patients in the community. An exciting opportunity to join the newly created Community Triage Service within Cardiff and Vale University Health Board. We are recruiting to a Band 6 Clinical Nurse Specialist post and offer this to clinical staff with good IT knowledge and skills, that are keen to work in a busy, friendly and developing integrated single point of access for community service referrals. Ideally we would like you to have community/primary care experience. The right candidate for this post will need to display excellent communication skills, problem solving and documentation with attention to detail and the ability to manage challenging and changing situations. They will require experience with managing deteriorating patients and have good clinical experience. They will be expected to work flexibly across the Cardiff and Vale bases and be available for home visits. They will also need to have a willingness to learn and be involved in service developments. The post will involve working in partnership with the Vale Community Resource Se rv ice and collaboratively with related specialities across the UHB. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. "}
