We are seeking a Community Health Care Assistant to support an efficient, effective, and quality adult community Night nursing service in Guildford and Waverley. You will deliver delegated health care activities working alongside a Registered Nurse including wound care, Palliative care and catheter care to people in their homes. The post is working between Monday to Sunday between 8 pm and 8 am Procare offer Agenda for Change Terms and Conditions and NHS Pension. Procare are committed to supporting the development of all employees to their full potential and all employees working within the Procare Community Services adult community nursing service will additionally have access to the staff benefits, learning and development opportunities offered by the Royal Surrey NHS Foundation Trust. The post holder will be required to work alongside a registered nurse overnight in the community setting, delivering nursing care to people in their own homes. You will deliver delegated health care activities working alongside a Registered Nurse including wound care, Palliative care and catheter care in the community setting and at times will work without direct supervision under the direction of the Registered Nurse. This post holder will need to ensure that all relevant policy, protocols, guidelines, and Standard Operating Procedures (SOPs) are followed. You will be a good communicator, calm and have a professional manner. You will be able to work methodically and independently whilst being a good team player. The post holder will need to be able to use technology to record patient visits and be able to travel to patients homes across Guildford and Waverley. You will need to be willing to undertake the health care assistant certificate as required. Job Summary: To deliver care in accordance with national and local priorities, policies, procedures, and guidelines and as delegated by the Registered Nurse within the Night Service Job Purpose: The post holder will be working alongside a Registered Nurse overnight to perform a range of nursing activities in the community setting for patients at home where the health and wellbeing of the individual would be compromised by travel. Activities will include wound care, Catheter care, blood glucose monitoring, assisting with Palliative Care . "}
