The successful candidate will manage a caseload of children and young people with cardiac conditions through to transition to adult services. The post holder will work with the child and family, and wider multi-agency team. Routine responsibilities include the support of Cardiac patients across the community service. To act as a point of contact for families, multidisciplinary team, other hospitals and other agencies in the community for specialist advice and information. "}
