In line with the Five Year Forward Plan for Mental Health and an exciting transformation program of services, we are expanding our existing Home Based Treatment services. Greater Manchester Mental Health Service are recruiting a Band 7 Mental Health Practitioner at our Central Manchester Site to Lead the Primary Care Urgent Referral and Assessment pathway within the team. The successful candidate will lead on this new pathway within the HBT team and support the development of the processes and procedures to establish this fully. The post holder will be making independent decisions within this role but would have the wider HBT team for support when required. You will be actively supported to develop as a mental health professional, building on your existing experience and knowledge. Your learning will be further supported by the fact that you will be working within an experienced multidisciplinary team among experts in a number of different fields involved in wider research and service development agendas Key duties willinclude: Job Summary/Purpose The post holder will: 1. Lead responsibility for overseeing development of the primary care urgent referrals pathway. Taking and assessing urgent referrals from primary care partners that previously sat with CMHT to determine the most appropriate patient pathway. 2. Provide clinical leadership, supervision, support and consultation to staff in the Home-Based Treatment Team in line with clinical pathway as outlined within the operational procedure. 3. Manage and evaluate a caseload in line with relevant clinical pathway as outlined within the operational procedure and will include: o Overseeing and supporting the referral pathway into the teams o Assessment o Formulation o Therapeutic intervention as required o Signposting o Close working with all colleagues o To be responsible for conducting clinical audits for referrals received from primary care. 4. Act as an assessor undertaking bio-psychosocial assessments. 5. Take responsibility for resource allocation and lead the development of the pathway with other community services. 6. Deputise for the Team Manager in his/her absence and take on any delegated management duties as required. 7. To participate in the bronze on call rota this is subject to approval. "}
