To provide a high quality ward based clinical pharmacy service, rotating through 4 month rotations in surgery, medicine, complex care and acute admissions. This includes reviewing drug charts for safety and efficiency, drug history taking and medicines reconciliation, medicine supply, patient medication consultations, medicines optimisation services, discharge medication service (DMS) and any other activities associated with medicines management.
