Identify patients who have not been issued anticoagulant/lithium/steroid/antiplatelet cards and ensure read code into the notes 4. Health Innovation Network (HIN) Project e.g. BP@home, lipid management a. Identify patients and risk stratify using UCLP search templates on EMIS b.
