In Addition To The Astley Cooper Dialysis Unit At Guy's, We Also Have Satellite Dialysis Units Located At Borough SE1 Camberwell Lewisham New Cross Gate Sidcup Tunbridge Wells We're looking for a dedicated and compassionate Renal Social Worker to join our team. This role involves providing psychosocial support to patients with chronic kidney disease and their families, advocating for their needs, and ensuring access to necessary resources. The successful candidate will work closely with the multidisciplinary team and make regular visits to dialysis units. The post holder will be responsible for the assessment, planning and coordination of social care provision, working with the multidisciplinary team and external agencies to deliver an effective social work service to patients and those close to them at all stages of the chronic kidney disease pathway The post holder will ensure the safety and wellbeing of patients and those close to them by the delivery of statutory social care The post holder will assess need and arrange delivery of social care packages for a patient caseload. This post is set within the renal and transplant service of Guy’s & St Thomas’ NHS Foundation Trust. The post is based at Guy’s Hospital and involves travel to kidney treatment centres in SE London and Kent.
