Supporting GP practices by conducting basic health services such as flu/covid-19 vaccinations and health checks Conduct patient assessments and where required, signpost patients to the appropriate service Supporting the implementation of PCN, local and national policies such as those aimed at reducing health inequalities Care Coordinators play an important role within a PCN (Primary Care Network) to proactively work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services. Care Coordinators will be based within our main hub at Goldsmiths Community Center and the option of occasionally working from one of our practices locations. We provide a high quality service to all our patients and practices and believe in exceptional patient care. There will be innovative and exciting services launching which will change the way we provide services to our patients. The Care Co-ordinator is the link between the patient and the NHS. They work closely with our patients to help them improve access, health outcomes and engagement with their health providers. Sevenfields PCN has higher than average levels of deprivation and BAME populations; patients have significantly poorer uptake of child immunisations when comparing with national targets. Practices have a high prevalence of diabetic patients; NHS Healthchecks are important to highlight early intervention for pre diabetic patients. Sevenfields PCN is committed to tackling health inequality and our care coordinators are a vital part of this work plan. "}
