We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included. This is a part-time post of 30 hours per week. The shift pattern for this post is variable, Monday to Friday. Please note the salary is Pro Rata for part time hours. This post requires PVG membership/Disclosure Scotland check. In order to apply for this position, you must have a Live PIN with the NMC UK. Working as part of the Discharge Team you will provide specialist discharge advice and intervention to all staff, patients, relatives and carers for all directorates within the Acute Services Division. The Discharge Team also includes Discharge Lounges and the Acute Homeless Liaison Team. The Discharge Team is a board wide service within the Acute Adult General Hospitals. A first level registered adult nurse on the NMC register with significant post registration experience in acute and/or community setting. Applicants must have band 6 experience. Proven management experience as well as an excellent working knowledge of discharge planning in acute care is essential.
