Prescribe cost effectively and according to best practice and provide prescribing information and advice to Pharmacists, Shared Care GPs and hospital teams. Support colleagues (Dr, IP) through individual clinical reviews as requested to support discussion and the development of a safe and effective treatment plan Deliver health promotion and harm reduction interventions in a client-centred manner (e.g. safer sex, contraception, nutrition and healthy living). Liaise with all agencies delivering services to or sharing care of clients including but not limited to sharing care with GPs for Service Users in Shared Care Services. Lead on complex appropriately assigned alcohol detoxification assessments for individuals who have decompensated liver disease and tolerance to diazepam. Provide prescribing information and advice to Pharmacy and Shared Care GPs. Review service users with complex physical co-morbidities including (but not exclusively) decompensated liver disease, the older population, end stage COPD, end of life. Working with wider partnerships in outside specialist services. Participate in external and internal subgroups and take responsibility for production or coproduction of plans and reports, as requested. Work with service users with complex poly pharmacy usage including illicit, over the counter (OTC) and prescription only medication (POM). Undertake doctor only specific clinics as needed for the service. This will include the oversight and of prescribing for those presenting with complexity, and off-formulary prescribing. Undertake the initial review, triage and ongoing support of complex dual diagnosis/presentation including pregnancy and young people. Provide specialist advice in the management of misuse of POM opioid analgesia to primary care colleagues and wider internal clinical team. Be an active member of the multi-disciplinary team (MDT) and peer group supervision, taking part in MDT/complex case review meetings and present as appropriate to the regional MDT meeting as needed following consultation with the clinical services lead.
