We are recruiting to our teams who will work with patients in their homes across the West Sussex communities to avoid hospital admissions and support those being discharged home. This is an amazing opportunity to become a member of the team as a Care Coordinator working together to help patients stay in their homes, making sure they avoid unnecessary hospital admissions and supporting them through the first few critical days after they leave hospital. The focus of this role is providing prompt and efficient co-ordination and administration support to a multi-disciplinary team (MDT). We Are a MDT Working Flexibly To Provide a Seven-day Service, 365 Days a Year, Between 8am And 8pm. We Deliver A rapid response to people in the community who are experiencing acute episodes of ill-health. A supported discharge service to facilitate timely hospital discharges. Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living. The team works closely with other services within Sussex Community NHS Foundation Trust, as well as our acute and community partners, GPs, South East Coast Ambulance Service (SECAmb), adult social care and voluntary and third sector organisations. In this role, you will be the point of contact for service users, their families, and carers when information on their crisis and short-term care support is required. This will include enquiries about anticipated visits, providing updates, and listening to their concerns. A key element of your duties will be processing and administering referrals using criteria guidelines and seeking advice from relevant team members when required. You will sign-post these referrals to MDT members and other services as appropriate while monitoring, tracking, and reporting on progress within the patient pathway. This will involve escalating any issues or delays to the Operational Team Lead for appropriate action. We are the main provider of NHS community services across East and West Sussex.
