About The Role You would be working with service users in their own homes, in the community and sometimes within residential environments. You will be assisting service users with all aspects of their daily lives. Each individual is different and requires unique care which is provided by individualised care plans. You will assist people with planning for their future and to prevent the effects of social isolation and loneliness. You will be working with others and building strong relationships with your team and service users. Supporting people to reach their full potential and lead independent lives. You will be creating and adapting care plans to ensure they are most beneficial to current circumstances and resources available.There is a mixture of shift patterns available with day shifts being the primary type of shift worked.
