You will be part of an established team delivering the Enhanced Care in Care Home (EHCH) service for the 16 Care Homes in the PCN Working under the direct supervision of the PCN Nurse and strictly in accordance with PCN EHCH Guidelines and protocols. Your duties will include Conducting 6 monthly care plan reviews. Ensuring all residents have a Personalised Care and Support Plan (PCSP) Administer Influenza and COVID vaccinations under appropriate PSD (vaccination training will be provided) Working within and supporting a multidisciplinary team (MDT) approach Phlebotomy is desirable but not essential (training can be provided) Hours are 9am -2pm Tuesday - Friday "}
