This will include working alongside the PCN Pharmacy team and GP supervisor to support our Care Home residents. Responsible for managing and running long-term condition clinics, with the possibility of running these clinics in our Extended Access service. As required, be able to prescribe for the caseload of patients within your proven scope of practice. Using your knowledge, you will provide information and advice on prescribed or over-the-counter medication on medication regiments, side-effects, and interactions. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. Collaborate with other members of the PCN including doctors, nurses, and pharmacists, accepting referrals, using clinical reasoning skills to recognise when referral to them for specialist care may be required. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associate with patient health care and registration with the practice. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
