An exciting opportunity has arisen within our well established Integrated Childrens Community Nursing team to introduce a new post into the Complex Care element of the team. The post holder will support the Advanced Clinical Practitioners (ACP) to develop and deliver an innovative specialist service for children, young people (C/YP) and their families who require specialist care. This will be a challenging role that includes offering sensitive support, advice to families & C/YP who have Complex Health Needs supporting the (ACP) and the Childrens Community Nursing Team (CCNT) in supporting the clinical assessment and prescribing for acute episodes and ongoing after care. The post holder will act as a point of contact for families who remain on ACP's/CCNT's caseload. They willprovide specialist information, advisory and training resource to schools and families and other voluntary and statutory service providers. They will develop links between disciplines and agencies to ensure a seamless services for clients.
