The Integrated Community Services Division includes Integrated Discharge, Intermediate Care, Neighbourhood’s (including Community Nursing), Falls and Frailty, Treatment Rooms, Diabetes, Endocrinology, Specialist Weight Management, Bladder and Bowel, Homeless support, Treatment rooms, Anti-coagulation, Community Neurology, Health Improvement, Allied Health Professionals and End of Life care. To work as part of the 24-hour community nursing service, under the direction of the district nursing sister, providing high quality holistic nursing care to patients. The staff nurse role carries delegated responsibility for the assessment, planning, implementation and evaluation of evidence based nursing to patients in their own homes and other community settings.
