We are seeking enthusiastic and highly motivated people to join our fantastic PCN team. We will provide full training for the role. The Care Coordinator will be directed to undertake a key role in helping to drive improvements in health outcomes for patients through the delivery of early diagnosis and preventive work with a specific focus on improving the uptake of Health promotion & screening. PCN3 consists of 7 GP practices with a population of over 70,000 patients. We enjoy excellent collaborative working relationships. Our practices are all high-achieving and innovative training practices, and our ethos is to develop our PCN with a focus on high quality, holistic patient care. We have a strong PCN leadership team. We have a rich diversity of PCN staff who aim to provide a personalised care approach for our patients, including Administrative Ops Team, Pharmacists, Nursing Associate, First Contact Physiotherapists, care co-ordinators, many other AHPs working alongside our GPs, Nurses and extended practice teams. We do not provide any form of sponsorship or work visas Main duties of the job The role will work closely with the PCN Head of Business Development & Transformation, Clinical Director, PCN Team leader, PCN Administrative Ops team, Clinical Leads, GP Practice Managers, PCN Practice Managers. The role will have a focus on the Coordination & facilitation of workstreams identified by the PCN. To work with hard-to-reach groups to support and engage participation in screening. Actively follow up those patients who have failed to attend for screening, to achieve improvements in compliance.
