An exciting opportunity has arisen for the post of a Community Support Worker in the Adult Community Mental Health Team. The service is based at The Ellis Centre in Scarborough and supports patients in the community with their mental health recovery. The role involves working with people in their homes , the community clinics and also providing inreach to patients who are admitted to hospital to support their discharge home. The post holder will work as part of the multi-disciplinary team delivering high quality patient focused care in a community setting. The role will involve improving the lives of people with mental health issues and the impact of their condition through the delivery of excellent services to promote recovery and wellbeing. It will involve promoting patients independence and also offering some physical health interventions. To be compassionate in meeting the needs of patients and their carers. Maintain safety, privacy and dignity of all patients in the delivery of patient centred care, recognising and respecting differences including spiritual and cultural beliefs. Responsible for recognising the signs of patient harm, abuse or neglect, and follows correct safeguarding policies Undertakes delegated activities within level of own competency, and with due regard to risk assessment, to assist with the implementation of intervention plans as part of the multidisciplinary team. Completes patient assessment tools and accurately summarises within patient care records. Identifies changes in patients' presentation and acts, reports and records appropriately. Accompany and provide support to patients in a variety of community settings, as delegated and dependent on current risk assessment.
