Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system. The post holder will be critical in supporting the delivery of the new Primary Care Network Contract Directed Enhanced Services (DES) specifications and will help lead improvements to maximise safe, cost effective best practice to improve the quality of patient care. This is an exciting opportunity to be part of our well established and experienced pharmacy team working closely with GPs and AHPs across the PCN and the wider locality. The post holder will work within their clinical competencies with ongoing support and supervision from the PCN Clinical Pharmacist as part of a multi-disciplinary team.
