They will help support the repeat prescription system and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s). The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. The clinical pharmacist will organise practice-based meetings with local community pharmacists to improve collaborative working and explore how to best support patients to access and get the best from their medicines. We recognise the importance of Clinical Pharmacists in Primary Care and the key role that they have is supporting the delivery of the Network Contract DES and Quality Outcomes Framework (QOF). Delivering a range of health interventions at Practice level, including developing and managing medicine optimisation services within Primary Care, systems for safer prescribing, structured medication reviews (for example of patients with polypharmacy), reviews of patients with long-term conditions that fall within their competency, withdrawing medicines no longer needed through NHS led programmes. For Enhanced Health in Care Homes residents will benefit from regular clinical-pharmacy-led medicines reviews. The Clinical Pharmacist plays a crucial role in our general practices and is of upmost importance to our local patient population. It serves as a cornerstone in our pursuit of enhancing health outcomes. The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a senior clinical pharmacist who will develop, manage, and mentor them and by a GP who will provide clinical supervision as well as a line manager. To provide clinical leadership on medicines optimisation and quality improvement as well as managing aspects of medicines safety and antimicrobial stewardship.
