An exciting opportunity has arisen within the memory assessment service in Wandsworth. We are looking to recruit a highly motivated and experienced band 6 memory assessment nurse, with a passion to care for people with dementia. The team sits under the umbrella of the Wandsworth Older People Services, which consist of four teams. The memory service offers a responsive and accessible service with most referrals from Primary care, aiding the early identification of dementia, by offering a full range of diagnostic services and holistic assessment of needs. The service also offers initial advice and pre and post diagnosis counselling support to people confirmed with diagnosis of dementia. The service will also develop a care plan with the individual and their carers including information and signposting to appropriate services for the client and carer to manage their care more effectively and thus enable planning for the future. The Post holder will: To provide high quality initial assessment and carry out investigations for adults with memory problems, leading to diagnosis. The post holder as part of an integrated, multi-professional team will undertake a review role for a defined caseload of clients with memory problems. The post holder will develop a dedicated collaborative care plan that includes signposting to the appropriate support services and agencies for the clients and carers to enable them to plan the future and responsible for reviewing this in accordance with the service spec.
