We also will be working on the Transitional Care ward based within the postnatal ward on a rotational basis as an extended role in our service to provide continuity of care and encourage parental bonding whilst reducing length of hospital stay for Term babies. The post holder will provide support to members of the team and carries responsibility for the assessment of care needs, the development, implementation and evaluation of programme of care and setting standards.
