Enhanced Support & Recovery at Home (East) Havant & Gosport Band 2 Southern Health NHS Foundation Trust plays a key role in supporting the discharge and ongoing care of patients from our local acute hospital and admission avoidance. In order to improve the patient experience, we have been asked to provide support to a programme of working relating to Medically Optimised Fit for Discharge patients in the South East area of Hampshire. The ERS@HOME team currently have the opportunity to recruit to expand the service. This means we have a number of exciting patient focused roles available to meet an increase in patient numbers so if you have the skills and aptitude to really do something wonderful today them look no further. The multidisciplinary Enhanced Recovery and Support at Home team based in Gosport and our satellite office in Havant are looking to recruit Health Care Support Workers to become integral part of our rehabilitation and re-enablement service covering a wide area. You will be part of a busy team providing varied day to day, patient focused short term packages of care working eight hour shifts over seven days between the hours of 8am and 9 pm. You will support our extensive team of OT's, physiotherapists, and nurses in providing community-based support, although not exclusively, to the over 65 age group within their own homes. Access to a motor vehicle and a clean driving license are essential for this role. Mileage will be paid monthly; uniforms and PPE are provided.
