We are currently recruiting to these two exciting opportunities: One permanent band 6 and one Band 6 rotation post (8 months rotation for internal candidates only) Both roles will be based across Home Therapies, therefore including Peritoneal Dialysis and Home Haemodialysis. We are looking for applicants with a willingness to learn and a passion for providing high quality patient care. The applicants do not have to be fully competent in these modalities, full training will be provided. You will play an intrinsic part of a dynamic team ensuring the teaching and co-ordination of care for our patient and families dialysing at home. You will work in partnership with our welcoming, supportive and expert team of consultants and CNSs across the outpatient service. Main duties of the job The post-holder will work across the home therapies service.Where mentioned home therapies and home dialysis relates to the care of patients on either home haemodialysis or home peritoneal dialysis. - Provide appropriate care, advice and support for renal patients and families - Provide appropriate information and sign posting to patients and families. - To promote home therapies, where appropriate with children and families - To provide information, clinical advice and support to children in chronic and end stage renal failure and their families who may require dialysis in a timely manner. - To train suitable families to be able to independently provide safe and effective dialysis within their own home. - To safely discharge children into the community, ensuring they have and understand their treatment parameters. - To be accessible to children and their families on home therapies to provide information, clinical advice and support. - To act as an advocate for the child and family on home dialysis - To ensure the provision of psychosocial care of children and their families -Working closely with members of the MDT both within the trust and outside agencies, to ensure patients are fully supported within the community. - Maintain open communication with the multi-disciplinary team in respect of the child's clinical condition and whole family needs.
