You will be a central co-ordination point for local multi-disciplinary activity, ensuring high quality customer service. You will work alongside the Care Community Team and partners including local authority GPs and the voluntary sector, to support patients within the care community. This post is full time - 37.5 hours per week. Good interpersonal and communication skills. Co-ordination of multi disciplinary meetings. Management of of daily meetings. Liaising with multi-disciplinary team members. Signposting team members and patients/carers to relevant services. Working days are Monday - Friday, between the hours of 9.00am - 17.00pm. CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
