You will be expected to provide 1 to 1 patient observations, assisting with individual therapy goals, being involved in rehabilitation through holistic and patient centered care. You must be able to demonstrate empathy, compassion and the ability to cope in challenging situations. You will work closely with the Complex Medicine multi-disciplinary team (MDT) and must have a desire to make a difference to vulnerable patients who require additional observations and intervention whilst in our care. The post-holder will be working on the wards. The post holder will be responsible for the delivery of holistic care and patient observations including distraction therapy, reminiscence therapy, personal care, support with nutrition / hydration, mouth care and prevention of deconditioning. This will be with the support and the guidance of the Complex Medicine MDT.
