The role requires clear communication between the Hospital Discharge Team, the Discharge to Assess Hub and the wards to ensure safe timely discharge of all patients . Main Duties And Responsibilities To support the Discharge Team in facilitating timely discharges for complex patients Attend Whiteboard rounds and ward rounds, identify actions required and actively manage agreed discharge plans for patients Assist in the completion of discharge check lists to ensure accurate discharge planning is evident, under the direction and supervision of the registered nursing staff Transfer patients to the discharge lounge if required We hold a unique position in the Stockport community as the provider of healthcare and we are one of its largest employers.
