Working within our integrated team, you will undertake duties involving the delivery of nursing services using a person centred approach. This involves providing quality care to patients in their own home, or patients in long term care settings, avoiding unnecessary admission to hospital. As the role involves visiting patients in their own environment, access to transport for work purposes is essential. "}
