Contribute to the communications and marketing strategy develop resources. Contribute to research, data collection and service development. Be up-to-date with a range of health improvement topics through competency based learning. 37.5 hours per week. An exciting and unexpected position has arisen to join the FUSE Team data driven innovative service at Livewell Southwest. As a valued member of the FUSE Team, you will be contributing to the NHS Long Term Plan to improve public health, de-escalating emergency admissions and reducing pressures on NHS resources. In collaboration with multi-agency teams, stakeholders, and voluntary organisations you will be working as a key practitioner to co-ordinate health and social care for clients with complex needs. FUSE values a strength-based approach to meeting holistic needs and aspirations of clients, reaching beyond medical, psychological, and social needs. You will have opportunities to contribute to public health research and population health data interpretation and service innovation.
