A full driving licence with business insurance and access to a car for work purposes. This role is for an end of November 2024 start date due to staff member retirement Providing general and specific care to adults within competencies Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of care including nursing, social and therapy based rehabilitative care to patients in liaison with senior staff. Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients’ own homes. Being responsible for the completion and potential delegation of non-clinical tasks within the team. Improve the care and support of patients in the community, so patients can remain safely in their own home for as long as possible Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery Willingness to further develop your clinical expertise in a range of areas.
