In West Kent, Kent Community Health Foundation Trust is now working closely with our GPs and the Primary Care Networks to provide health and social care tailoredaround the individual.With the creation of the new Edenbridge Memorial health Centre, an exciting new opportunity has arisen to support the local residents. The development of this community hub will link key services together to provide a proactive frailty model that optimises the ability to closely work with the GPs, as part of a joined-up community response with urgent care, and community and complex care nursing teams. It will also utilise the benefits of having a wellbeing day centre and health and wellbeing team working together to wrap care, support and education around individuals and their families/carers.Therapy services will play an integral role within this proactive care model and we are expanding our existing therapies team to support the creation and delivery of this new innovative project alongside our existing clinical leads.We have the opportunity for an Occupational Therapist to join us and help deliver this new service development alongside our existing domiciliary proactive pathway.The role will be supported by the Community Rehabilitation Team and existing MDT Therapy Provision. These multi-disciplinary therapy streams will enable the applicantto access clinical supervision, training and a robust peer network and be part of a well-established team for professional support. We are looking for someone with excellent holistic assessment and clinical reasoning skills, and a wealth of clinical experience to create proactive and personalisedcare plans. There will be a mixture of work based on site at the health centre in clinic and day centre settings, as well as community visits to patients in their own homes across thelocality.You will need to be able to build excellent working relationships with colleagues within the health centre and the wider KCHFT services, as well as our health and social care partners.Strong communication skills and innovative thinking will be required for engaging patients, staff, and families to put plans into action, and delivering training as required.You should also have a keen interest in service development.All our Band 6 therapists are expected to support undergraduate and apprenticeship clinical placements.All our Band 6 Therapists are expected to supervise junior therapy staff and / or therapy assistants as part of their role. As an autonomous practitioner delivering community care, it is a requirement of the role that you are able to drive and have access to a car (subject to the provisions ofthe Equality Act 2010). "}
