The Frailty Practitioner service works with older adults in the community living with long-term conditions and frailty combined with admissions to hospital. The aim of the service is to optimise clients’ health & medical treatment whilst providing them with the opportunity for advance care planning. You will be motivated & organised, with the ability to work alone and as part of a closely integrated team. You will have strong communication & teamworking skills, a knowledge of the care of older adults and a willingness to learn about Frailty. You will work alongside Clinical Support Workers and Frailty Practitioners (Nurses and Physiotherapists) to assist with clinical assessment and to measure & record clinical information. You will provide frail patients with health education, health monitoring and support. You will also be required to take part in team development initiatives. The successful candidate will work across H&R & EHS community locations. The office base is in Eastbourne/Hastings. You must have use of your own car to travel independently whilst at work and to carry small items of clinical equipment as required.
