We provide multidisciplinary rehabilitation for patients with complex disabilities which are neurological in origin, including conditions such as Stroke, Multiple Sclerosis, Traumatic Brain Injury, Parkinson’s disease, Motor Neurone Disease and Spinal Injuries. We are an adult service providing rehabilitation to individuals aged range 16 to 64. Currently, Patients 64 & above are accepted if considered appropriate following the team’s assessment. We are seeking a motivated, caring, and dynamic rehabilitation Nurse with experience in stroke, to apply for this exciting opportunity to work in our team. The Nurse will work closely with the Multidisciplinary Team consisting of Consultants and Doctors, Matron, Ward Manager, Staff Nurses, Health Care Assistants, Therapists, Dieticians, Pharmacists, Psychologists and Domestic Staff. The Nurse will assist the Ward Manager to provide nursing care to, and support recovery of, patients to meet their rehabilitation goals, focusing on the needs of the patients rather than the illness or condition. Also work closely with other Allied Health Professionals and Social Care services, to maximise patient’s recovery, building confidence to regain their control and improve their quality of life. Preparing patient care plans. Implementing plans of care and reviewing their effectiveness. Observing and recording the condition of patients. Checking and administering drugs and injections. Setting up drips. Assisting with tests and evaluations. Carrying out routine investigations i.e. Testing urine Responding quickly to emergencies. Organising staff and workloads Teaching skills to student and junior nurses. Making ethical decisions related to consent and confidentiality. Escorting patients. Be familiar with the steps taken when medical or untoward incidents arise, and act accordingly, entering incident on Datix. To contribute to the maintenance of high staff morale and good working relationships with all members of the multidisciplinary team.
