Main duties of the job The successful candidate will be supporting the Shropshire Rural Alliance PCN to achieve the successful delivery of our Direct Enhanced Service (DES) specification, specifically working with our Care Homes, delivering Chronic Disease clinics, and supporting with our Extended Access service. The post-holder will be supporting the PCN practices by working closely with the Clinical Director and Management Team. The post-holder will be expected to work across all of our practices in the Shropshire Rural Alliance PCN. About us The following practices are members of the Shropshire Rural Alliance PCN: Clive Medical Practice - High Street, Clive, Shrewsbury, SY4 5PS Knockin Medical Centre - Knockin, Oswestry, SY10 8HL Pontesbury & Worthen Medical Practice - Hall Bank, Pontesbury, Shrewsbury, SY6 0RF Shawbury Medical Practice - 47 Poynton Road, Shawbury, Shrewsbury, SY4 4JS Westbury Medical Centre - Vennington Road, Westbury, SY5 9QX Job description Job responsibilities The Advanced Nurse Practitioner has the following key responsibilities in delivery health services at Shropshire Rural Alliance PCN. There may be, on occasion, a requirement to carry out other tasks. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated diagnosis. Clinically examine and assess patient needs from a psychological perspective, and plan care accordingly. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition. Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods in to a management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Work with patients to support compliance with and adherence to prescribed treatments. Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of care. Clinical responsibilities: The post-holder will assess and triage patients, manage undifferentiated, undiagnosed condition and as appropriate provide definitive treatment (including prescribing medication following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team. The post-holder will need to demonstrate complex decision making to inform the diagnosis, investigation, and complete management of episodes of care within a broad scope of practice. Personalised Care: The post-holder will actively take a personalised care approach and population centered care approach to enable shared decision making with the presenting person. This will include using a high level of interpersonal and communication skills to communicate effectively with patients and multi-disciplinary professionals, including communication sensitive and complex information concerning patients' medical conditions. Cross organisational working: The post-holder will effectively communicate at all levels of the organisation, with a variety of health professionals, users, and carers to provide the best health outcomes for patients.
