Our Benefits About The Role This is an exciting opportunity to be part of our Children with Disability team as a Family Support Worker. In this role, you will work closely with children with disabilities and their families to address multiple and complex needs, improving family functioning and resilience, reducing the need for statutory interventions. You will develop, deliver, and evaluate creative, robust, and outcomes-led packages of intervention and support for children, young people, and their families, based on whole family assessment. You will provide an integrated response to meeting the needs of young people through collaborative working with colleagues, partners, young people, and their families/carers. As a Family Support Worker, you will hold case responsibility, manage and prioritize a caseload, working alongside children with disabilities and their families in their homes and the community sometimes until the child turns 18. You will support families to achieve change, build resilience, and reach a point where the young person can function effectively with the least restrictions. You will also support families to access and engage with mainstream services, raising awareness of services available, taking families to appointments and meetings, supporting parents to complete forms, and advocating on their behalf. This role requires flexibility to meet the needs of families, including occasional early morning and evening working. You will occasionally provide periods of intensive direct support for children with disabilities and their families in crisis. We look forward to receiving your application! About The Team You’ll Be Working In The children with disabilities team provides social work services to children with disabilities and their families. It is a rewarding and fast paced work environment where you will be supported by experienced managers to be part of the team which delivers early help, Child in Need, children looked after and child protection services to the diverse community of Hounslow. The team sits within the Children and Adult Services directorate and consists of a social work team, a short break team, our business support officers and the Family Support team. We are co-located with the SEND EHC team and have close working relationships with the Childrens Community Nursing Team, our local schools, and NHS therapy services. You will be supervised by one of the assistant team manager of the children with disabilities team and you will have three family support worker colleagues who will support you grow and develop our new family support team. We are based in the council main office, Hounslow House, in the heart of Hounslow and serve all residents of the borough.
