Toassist and perform routine nursing tasks related to patient care, such asroutine BP checks, ECG, suture and clip removal, BMI and lifestyle advice, asdirected and agreed with line manager, registered general nurses and GPs. To support patients to improve and maintain theirmental, physical, behavioural health and wellbeing by using behaviour changeinterventions, such as undertaking health checks (physical and mental), smokingcessation services. Tosupport the RGN with the holistic assessment and planning of wound care plansfor patients with chronic and acute wounds. Toimplement agreed wound care plans, following local and national evidence-basedguidelines, inclusive of the treatment of minor traumatic wounds within ownlevel of competency. To recognise any deterioration of wounds or infection andarrange early review with PN/GP or specialist as required. Carryout Chronic Disease Management annual reviews delegated by the Practice Nurseor senior colleague. Supportthe Practice Nurse with the management of patients with long-termconditions/chronic diseases, using a holistic patient centred approach. Assistin and perform routine tasks related to patient care as directed by thePractice Nurse GP or other member of the Primary HealthCare Team. Beable to undertake baseline observations and identify signs of improvement,report any deteriorations or concerns, e.g. pulse, blood pressure,respirations, oxygen saturations, peak flow, spirometry, blood glucose levels,ECG, assessing nutrition and hydration, participate in screening programmes andAnnual Reviews for example in patients with Learning Disabilities .
