We have busy and varied caseloads, and you will be visiting housebound patients in their own home. You will be well supported in Practice to enable you to develop a large range of clinical knowledge and skills. These provide integrated multi-disciplinary services, supporting individualised patient care and the needs of the local population The post holder will deliver Nursing Care as part of a team. You will support the District Nurse in the delivery of an efficient/effective nursing service. You will have responsibility for assessment, planning, delivery and evaluation of care whilst working autonomously as a lone worker. You must be able to travel throughout the locality in a timely manner, carrying equipment/staff with you. You must have access to a car and hold a current driving license.
