You will work closely with the primary care networks to ensure that Intermediate Care services are delivered in a safe and effective way; with the outcome of providing an alternative to hospital admission, enabling earlier hospital discharge and preventing early admission to long term care. You will also develop and maintain professional working relationships with the Care Homes commissioned to provide Intermediate Care bed-based care.
