This particular post will be aligned to the Lung Cancer Pathway The post has been newly created to support the trust in providing an effective, caring and compassionate service to patients to optimise their access to cancer services. It is expected that this role will work to support the clinician and patient through the patients journey, supporting a streamlined diagnostic and treatment if required . The Cancer Pathway Navigator will work within a designated cancer specialty whilst working in conjunction with diagnostics services, the multi-disciplinary team Cancer Pathway Coordinators and members of multi-disciplinary teams in primary and secondary care organisations, including working closely with the Cancer & Access Performance Manager. They will work autonomously as a member of the Cancer Pathways Team but will be expected to spend significant periods of time working in within the designated cancer pathway. It will be an essential component of the job to form close working relationships with the medical, nursing, and allied health professionals from all the Trust cancer multidisciplinary teams across sites and within Primary Care. The Cancer Pathway Navigator will provide training to all disciplines within the specialty on the National Cancer targets. Providing specific in depth knowledge of how this interprets for the specific specialty. The Cancer Pathway Navigator will provide a point of contact to GP’s and patients/carers advising and updating on patient pathway status with the specific aim of minimising any delays throughout each patient journey. Within the Trust, they are also point of access to receive notification of new, suspicious, or recurrent cancers which are identified following diagnostics, and an essential element of the role is to ensure the Cancer Pathway Coordinators and the relevant MDT are alerted and relevant appointments are secured. A key aspect of the role is to liaise with the patient or their carer to provide detail of next appointments and arrange any necessary tests prior to imaging. They will act as patient advocate to enable timely access to cancer services and will also follow up with the patient should they fail to attend. Some patients do have complex lifestyle which do require some persuasion to attend appointments/treatment. The Navigator will be a key pivotable role to ensure agreement and co-operation of the patients/carers throughout their pathway journey. The Cancer Pathway Navigator will provide support to the Cancer & Access Performance Manager in regard to 1-1’s and Appraisals of the Pathway Co-ordinators.
