Key Working Areas Primary Roles: - Medication reconciliation - Support the practices to achieve cost effective prescribing To provide advice and support as part of the MDT; to promote the clinically appropriate use of medicines at practice level. Carry out medicines optimisation tasks under direction of PCN Pharmacists - Working towards undertaking a patient facing role to ensure effective medicines use through shared decision making conversations with patients Day to Day Workload to include: Direct discussions with patients on a range of medicine related issues e.g. medication compliance and post discharge reviews Support pharmacists in medication reviews / deprescribing for care home patients Medicines reconciliation after hospital discharge and from clinic letters Manage shared protocols and liaise with pharmacist for more complex patients Support clinical pharmacist in structured medication reviews Support initiatives to reduce inappropriate antibiotic prescribing To carry out audits on prescribing as required Provide training and support to reception and administration teams Communications And Working Relationships: Within the PCN: Clinical Pharmacists Prescribers in each practice Practice staff Primary Health care teams External: ICB Medicines Management Team ICB Practice Pharmacists Community Pharmacists Pharmacists and other staff from neighbouring PCNs Secondary Care Nursing and residential homes Pharmaceutical Industry Key Challenge of role Influencing and implementing change in the management of medicines at practice level in line with the rapidly developing national and local agenda.
