We are looking for a full time pharmacist to join us working across our Moreton & Meols PCN and Wallasey Wellbeing PCN teams. You will join experienced teams that include GPs, Nurses, Health & Wellbeing Coaches, Care Coordinators, Social Prescribers & Paramedics to provide clinical pharmacy services to our local population. You will support the PCNs with the delivery of safe, effective systems for medicines optimisation, reducing medicines waste and maximising patient outcomes through structured medication review. You will contribute to new ways of working and using the skills that your role can bring to evolving PCN services. You will be responsible for promoting good practice in line with clinical and operational guidelines and will clinically support the pharmacy technicians with medicines reconciliation post discharge and other medicines projects. On a day-to-day basis you will spend time within our hubs and in GP practices to answer clinical queries from patients, local GPs and clinicians, trusts and other services such as care homes. You will be supported to build strong relationships with community pharmacy and trust pharmacy colleagues. Ultimately your role will be to support our patients to help them get the most out of their medicines and to play an active part in their own healthcare decisions. As part of the WUTH Integrated Pharmacy Service, this role is based across Moreton & Meols PCN and Wallasey Wellbeing PCN joint pharmacy team. There are opportunities to work with colleagues from the other PCN teams as well as having access to the Acute Trust specialists which helps you develop and better understand the full patient journey.
