The service operates across Wessex and Dorset, provided by a number of NHS Trusts, working in partnership with the Wessex and Dorset Tier 4 Provider Collaborative (W&D PC) to deliver a clinical model of evidence-based intensive home treatment as a safe and effective alternative to hospital admission. As a Community Adolescent Senior Support Worker (CASS) you will work as a member of a multi-disciplinary team which provides an alternative to Tier 4 admission and facilitates supported discharges from hospital. With team colleagues you will be responsible for delivering time-limited programmes of intensive home treatment to young people who would otherwise have been admitted. Young people requiring home treatment will have a range of needs commensurate with requiring inpatient services but be offered intensive home treatment as an alternative. The team work collaboratively with community CAMHS, children's services, colleagues involved in 'transforming care,' bed and case managers and local inpatient units; with inter-agency networking embedded in the service model.
