The Role The organisation is recruiting an Advanced Health and Wellbeing Practitioner to join their team which supports people with long-term physical and mental health conditions to self-manage and improve their wellbeing. The role requires an experienced practitioner with the proven ability to build successful relationships with local agencies and organisations within community settings and to support the development of practitioner colleagues, volunteers and peer workers. The successful candidate will also form part of the organisation’s Training and Consultancy team and will deliver external training in collaborative, outcomes focused and strengths-based approaches to health and social care colleagues across Scotland. Skills & Experience Be a health and social care practitioner with extensive knowledge and experience of supporting people living with long term physical and mental health conditions to self-manage. Have a background in collaborative, outcomes focused, strength based and trauma informed approaches and recognise the value of a generic, non-specialist approach to supported self-management. Be experienced in managing caseloads, waiting times, and supporting colleagues to achieve expected outcomes. Be able to cope well supporting vulnerable people who experience significant psychosocial barriers and health inequalities. Hold a firm belief that people have strengths, are resilient and are experts in their own lives. Have excellent solution focused coaching and group facilitation skills and experience of designing one-to-one or group-based support. Benefits A highly attractive work environment.
