We are looking for a Community Nurse to work in our Frailty Virtual Ward Service. Working across, Scarborough, Whitby and Ryedale localities, delivering care in patients home setting/usual place of residence, in line with the VW frailty pathways and national strategy. As a community nurse you need your own transport and to be comfortable working alone in patient homes. You will work closely with other community nurse colleagues and the wider MDT within our own services and other agencies including the acute hospitals, Hospice, General Practice, Social Services & the voluntary sector. We are a 24/7 service so there is opportunity for different types of shifts. Ideally applicants would have at least 6 months post registration experience and an interest in community nursing, frailty and ageing well. As a community nurse, you would be supported to develop into an autonomous practitioner,nursing patients with multiple complex health care needs. Main duties of the job There would be an excellent induction, support from Senior Nursing Staff, and a Preceptorship Programme for newly/recently qualified nurses. The service operates 7 days a week. As a Community Nurse, you will work within an integrated hub model of Community Nurses, Specialist Nurses, Community Staff Nurses, Physiotherapists, Occupational Therapists, Registered Nursing Associates, Assistant Practitioners & Health Care Assistants to meet the needs of the complex patient in the community and avoid admissions to the acute sector as far as practicable. This includes (once competencies have been achieved): Intravenous Antibiotic administration; End of life care as appropriate to service model Recognising and Managing the deteriorating patient; Complex and simple wound care Phlebotomy, sample collecting, swabs in line with process Point of care monitoring, remote monitoring digital platforms Utilising physical assessment skills and clinical nursing skills to complete patient assessments including falls, nutrition, pressure ulcers and the deteriorating patient. Person specification Qualifications And Knowledge Essential criteria Degree/diploma in adult nursing, must be willing to undertake mentorship training and responsibilities Part III or 13 of NMC live register An understanding of relevant clinical practice/standards within identified clinical area Desirable criteria Evidence of current CPD/formal qualifications at an advanced level e.g. degree or post graduate level or equivalent knowledge/experiential learning Experience Essential criteria Appropriate knowledge/experience of working in Community Nursing, in people's usual place of residence Understanding of caseload management responsibilities Willingness to undertake and complete preceptorship, facilitating the transition for newly qualified nurse booklet and core competency development programme.
